What is required to add a dependant to nhif

I need to add my daughter in the NHIF scheme. What is required for this to happen?

If your daughter is below six months then you are required to produce her birth notification. If she is above 6 months then her birth certificate is required plus passport photos.

You are required to visit any NHIF office near your area or Huduma Center (NHIF Desk) and fill and submit NHIF dependent addition form.
